CONTINUING MEDICAL EDUCATION (EACCME & ECM ACCREDITATION)

The Parathyroids 2010 scientific program will be submitted for accreditation by the Italian Educazione Continua in Medicina (ECM) and the European Accreditation Council for Continuing Medical Education (EACCME). EACCME credits are recognized by the American Medical Association towards the Physician's Recognition Award (PRA). To convert EACCME credit to AMA PRA category credit, contact the AMA. Delegates wishing to claim CME points must fill in the CME forms and questionnaire in the delegate bag and hand it at the registration desk at the end of the meeting.

CERTIFICATE OF ATTENDANCE
Certificates of attendance will be distributed after noon on Saturday, February 13, 2010 at the registration desk.

Registration applications received after Wednesday, February 3, 2010 will not be processed. Delegates should apply on-site for registration after this date.

LETTER OF INVITATION
If you require a letter of invitation to participate in Parathyroids 2010, upon request on dedicated section in Registration Form and one will be forwarded to you with your confirmation letter. Early registration is required.

CONFIRMATION

Confirmation will be sent by e-mail to all delegates after receipt of registration, accommodation details and payment. Registration applications and payments sent after Wednesday, February 3, 2010 may not be confirmed in written form since the advance registration will be closed after this date.

PAYMENT

A filled Registration Form, along with full payment, your credit card number (VISA or MasterCard only), must accompany your registration in order to be processed. Registrations without payment will not be processed. If you are faxing your registration, do not e-mail or you may be double charged. Asti Incentives & Congressi will only charge one night hotel accommodation in order to guarantee your reservation, balance must be paid directly to hotel on arrival.

INVOICE-RECEIPT
Asti Incentives & Congressi will issue invoice for registration and accomodation fees.
Hotel will issue invoice for the whole period of accommodation.
Delegates are asked to fill section 3 on Registration Form.

CANCELLATIONS & REFUNDS (REGISTRATION FEE AND HOTEL ACCOMMODATION)

Requests for refunds must be received by e-mail or by fax by Friday, January 8, 2010. No refunds will be issued for cancellations received after this date or for no-shows. All refunds will be processed after Parathyroids 2010, minus a Euros 30.00 administrative fee + Euros 15.00 accommodation fee.

LIABILITY AND INSURANCE

The registration fees do not include the insurance of participants against personal accidents, sickness, cancellations by any party, theft, loss or damage to personal possessions. Participants are advised to take out adequate personal insurance to cover travel, accommodation, cancellation and personal effects.
